Purpose and philosophy.
The Green River Valley Health Foundation (GRVHF) raises funds to advance integrated health care in Sublette County, Wyoming. These funds may be requested to support projects and programs that are in alignment with the GRVHF’s goals.
Limitations and eligibility.
Any nonprofit or government health care provider or health related service (i.e. GO SPOKES) serving Sublette County may apply for funding by filling out all sections of the GRVHF Funding and/ or Support Request Application. Applications must
- Address a specific need related to the health and/or wellbeing of Sublette County citizens
- Support something that is not funded through the general operating budget of the requesting organization
- Advance integrated health care in Sublette Page County, 1 of 2 Wyoming.
The individual submitting the request for funds must
- Be directly involved in the proposed project, program or service
- Be able to provide the GRVHF with initial results within 3-6 months of implementation
- Be able to provide the GRVHF with a complete accounting of the grant funding over the request’s life cycle.
The Foundation Health Fund does NOT provide funding for:
- Events
- Sponsorships
- Requests from individuals
- Requests in the normal course of operation at Health Care providers

*The GRVHF Board reviews applications in order to select and prioritize those submissions that meet the requirements defined below. By reviewing requests for support during monthly meetings, the GRVHF Board acts as the steward for focusing resources into action and impact. Grant applications are evaluated using the following criteria:
- Eligibility & Mission Alignment
- Available Funding
- Accuracy and Comprehensiveness of Application
Preference will be given to projects that are self-sustained over the long-term. These policies pproved by the GRVHF Board of Directors on June 8, 2017.
